  • A BMI over 30 or a BMI of 27.5 with comorbidities (eg High Blood Pressure, Diabetes)
  • Aged 16 years or over
  • A Hillingdon resident
  • Committed and focused on adapting to a healthier lifestyle
  • Willing to commit to a 12 week programme
  • Available to attend all weekly meetings

Patient Journey through the service:

  1. Let the admin team know which group date and time suits you best
  2. Be sent your zoom link for the group and your pre-program materials
  3. Have a friendly, introductory phone call with your facilitator prior to your first week.
  4. Attend your group at the same time and day each week
  5. Receive support each week via your group, your facilitator and our motivational newsletters
  6. Graduate as a healthier and lighter you after 12 weeks!

FAQS

How can my patient book onto the service? Patients can be referred into the programme via a GP referral.

How many days per week do the sessions run? We provide a wide range of session times and days. Both evenings and weekends sessions are provided to work around our patient’s commitments.

What does the course cover? Will I have to stick to a certain diet or specific exercises? The course is aimed to help equip patients with all the tools to lose weight in the way that suits them best. Your facilitator will help guide you to find the diet and exercises that you can start and enjoy for long-term lifestyle change.
