What is Hypertension?

High blood pressure, also called hypertension, is when the pressure in your blood vessels is unusually high. If the pressure is consistently too high, it puts a strain on your heart and could lead to strokes, heart attacks and kidney damage. Things that increase your chances of high blood pressure include being overweight, having an unhealthy diet, smoking and not exercising enough. 

What are the signs of high blood pressure?

High blood pressure does not usually have any symptoms. The only way to find out if you have it is to get your blood pressure checked. Lifestyle changes like eating a healthy diet and exercising regularly can help lower high blood pressure. Some people also need to take medicines.

What is Hypertension Plus?

Hypertension Plus is a primary care remote hypertension management and medication titration platform created by OMRON. This is being rolled out in Hillingdon with the Metocare and Celandine PCN being the first to piolet the service. This free service is to patients monitor and manage their condition remotely. Are you eligible and how do you join?

If you are eligible you will be contacted by the Hillingdon GP Confederation in partnership with your GP surgery via sms.

There is no need to contact your practice.

What will you gain from Hypertension Plus? 

The programme will be a great way to help you and your doctor agree on a hypertension plan and keep your blood pressure in a healthy range. You will also be able to do the following:

  • Record and share up to date blood pressure readings with your doctor via the app
  • Save time with convenient home monitoring instead of having to visit the surgery
  • Stay on the right medication plan, agreed by yourself and your doctor
  • Make faster progress towards your target blood pressure

Please be aware registering can take up to 15 minutes whilst it accesses your medical records from our clinical system.

 Stage Two

Once you have registered via the unique invite link or have been registered manually, the first time you access the application you will need to register again with your first name, surname and email address which MUST match what we have registered on our clinical system.

This is because the 1st registration is linking your details but does not create an account on the application.               

It may look like your phone has frozen and is stuck on the loading screen, please allow it to finish. The app has not frozen and is working in the background. Attempting a 2nd registration can prevent the initial registration from completing.

In this case you would need to contact us so we can manually register you on the system. You can contact us on 01895 544 227 and ask for the Hypertension Team.

 

Blood Pressure Machines

If you are without a blood pressure machine, you may ask your practice if they are able to loan/borrow you one. however if your practice is unable to do this then The British Heart Foundation sell approved blood pressure machines, please Click Here (please note that BP machines come with a standard medium cuff, if you need a larger cuff please Click Here)
